How To Upload Your Immunization Records, COVID Vaccine/Booster Records, Titers, and/or Exemption Forms to Your Reed College Student Health Portal

1. Log on to the Reed College Student Health Portal

2. To upload a copy of your official immunization record/s for verification of immunizations you have received, select Upload from the toolbar located on the Student Health Portal’s home screen.

*Note: *Official documentation includes: COVID vaccine cards received when receiving a COVID vaccine; a printout from your medical record portal, such as MyChart; State immunization registries, documentation from your medical provider, school immunization records, international health records (All international health records must be translated to English to be processed).

3. From the Upload screen, navigate to the Choose Document You Are Uploading drop down menu:

4. To upload non-covid related immunization records and titer lab results, select Immunization History (Scanned Immunization Records). Use this selection if you are uploading immunization records/titer lab results that are not COVID-19 vaccine/boosters documents or exemption forms.

5. Click Select File to upload a copy of your official immunization record/s.

6. To upload your Covid Vaccine/Booster documentation, select Covid Vaccine Documentation. Do not upload exemption forms under this selection.

7. Click Select File to upload a copy of your official immunization record/s.

8. To upload your COVID-19 Vaccination/Booster Exemption Form, select Covid Exemption Form.  Please use the Covid Exemption Form located on the Student Health Portal under the Forms tab.

9. Click Select File to upload a copy of your official immunization record/s.

10. To upload your Measles/MMR form and Oregon Health Authority’s College Vaccine Education Certificate., select Measles/MMR Exemption Form.

11. Click Select File to upload a copy of your official immunization record/s.

*Note: We will accept photos of immunization records.  All images and documents uploaded must be of good quality and easily readable.  If documents and photos are not legible or readable, you will be asked to upload a better copy before your immunizations can be verified. Any immunizations or testing for immunity performed internationally requires results translated into English before submission.

To check to see what immunizations we have on file for you and to print out a copy of your current Reed College Immunization Record, Select Immunizations.

Then Select Immunization History and scroll down to the bottom of the page.  There you can see what immunizations the HCC has on file for you.  You can also print a copy of that record.
